If you have made your initial appointment, you can preregister by clicking on the links below to download our New Patient Forms. Please print and complete the forms prior to your arrival. Please review our financial policy below. Please call our office if you have any questions regarding your account. This will help to speed up the new patient registration process and get you in to see the doctor faster!
Prescriptions:
Please call your pharmacy for all prescription refills a week prior to the end of your medication.
Insurance:
We accept most insurance. Please call to verify whether or not we are participating with a particular insurance company.
Hospital Affiliations:
HCG is affiliated with St. Francis Hospital and University of Connecticut Health Center, Hartford Hospital, ECHN (Eastern Connecticut Health Network) and have full privileges.
